Fill out our mail / fax order form and fax in to the number or mail to the address provided on the form.

Step 2
When you are finished shopping, print the Shopping Cart page and mail the printed Mail/Fax Form and the Shopping Cart summary page.

Do you charge sales tax?

We are headquartered in Panama City Beach, Florida and only residents of Florida pay a 6% sales tax. All orders shipped to addresses outside Florida are exempt from sales tax.
